5" ground manifold catastrophic failure in NJ

Got this in an email and I hope everyone checks their equipment on a regular basis for potential cracks and damage. Have you ever seen this before? What could have caused it: lack of proper care, wear and tear, dropping it on the ground, excessive high pressures? has yet to be determined. Here is one similar that I found online. I'm not sure of the brand they had in the pictures. Similar Manifold

Attached are some photos from a Manifold (street Hydrant) as we call them in
NJ..that exploded on Saturday night during Water Tender filling at a
multi-alarm fire. An investigation is underway to determine the exact cause
or what led up to the event. Two FFs in vicinity of the manifold were
treated for minor injuries.
