Not sure about the other question but most of it can be found in the stds. Eddie Moore atht e academy is a great source. So is the state.
As per your CSR scenario question, that is exactally the way we would have done it. As soon as I read the space configuration I was already thinking "Inch Worm." With the vertical drop and then horizontal travel, running the retrieval system from on top works, but the entry team using a spearate MA system to complete the horizontal move to the vertical shaft works fantastic if there is room to pull in the main vault. It gives you a direct line of sight to your patient and allows for better feel and control of the movement. Depending on the horizontal distance from the main vault, a Z-Rig anchored to the same place could also be configured giving your team a 3:1 in place of the 2:1 provided in your question while still giving control to the rescue team. If you have enough rope to set your traveling pulley at the pt. you should not have to reset the system before your pt. gets to the main vault.
Both scenarios (Your original plan & the plan used) get the job done. The joy of this stuff is that there is no one right way. I keep telling myself and my team members that"Just because I wouldn't do it that way, does not make it wrong!" Discussions like this are why I joined this website. Sounds like you have a decent team and equipment (I would love a second air system!) Keep it safe and always carry spare headlamp batteries!!!!!!
