5" ground manifold catastrophic failure in NJ

Got this in an email and I hope everyone checks their equipment on a regular basis for potential cracks and damage. Have you ever seen this before? What could have caused it: lack of proper care, wear and tear, dropping it on the ground, excessive high pressures? has yet to be determined. Here is one similar that I found online.
