How do other department log training records

We use a report that has the following, Date, type of training, detailed discription of the training, all that attend (and signed by each member), any equipment being used, lead instructor and assistant instructors. We also do a sheet for a roster. We had some problems of people saying that we did not train them on this, so we started having them sign the roster and drill report. Also we have the Batt Chief sign and the Training Officer sign.
