Where firefighters come to talk training

Does any one do QAs on fire reports to make sure they contain certain information for each type of call they respond to? For instance on a CO alarm, a structure fire, or a vehicle fire or extrication what info do you require to be included in the report? We are going to be reviewing all fire reports to make sure they contain certain info and if they don't include it then the report writer will have to add it to their report. All our reports are done on computer I'm looking for ideas of what other departments use for report writing criteria.

If you are doing your reports on the computer anyway, have you looked at Firehouse software by ACS Solutions?
That is what we are currently using now. I need ideas of what should be in the narrative of the report. I personally don't care for the computer generated narrative that you can pop up.For instance on a Illegal burn call the narrative should contain if the fire was extinguished and by whom and if a oral, or written warning issued or if a ticket was issued and what the violation was, Ex burning on a non burn day. Or on a CO call have down that the monitor wa zeroed prior to entering the structure and where you monitored at and what the readings were at different spots.
